Dr. Samuel Moulton is a fellowship-trained board certified orthopedic surgeon with expertise in sports medicine, shoulder replacements and minimally invasive arthroscopic surgery of the hip, knee, and shoulder. His treatments cover a wide range of conditions including hip labral tears, femoroacetabular (FAI) impingement of the hip, shoulder instability, rotator cuff tears, shoulder osteoarthritis, knee ligament injuries, meniscus injuries, and cartilage injuries.

Background
After receiving his BA in Biochemistry, Biophysics, and Molecular Biology from Whitman College in Walla Walla, WA, Dr. Moulton went on to complete his medical degree, followed by his orthopedic residency at Oregon Health & Science University (OHSU) in Portland, OR. During medical school, he spent a year focusing on his passion for sports research at the world-renowned Steadman Philippon Research Institute, then completed his fellowship training in sports medicine and shoulder surgery at the University of California San Francisco (UCSF). During this time, Dr. Moulton served as a team physician for the San Francisco Giants, Cal Men’s Basketball team, the University of San Francisco Women’s basketball team, and local high school football teams.
He has since authored dozens of publications on knee, shoulder, and hip conditions as well as textbook chapters, and continues to stay engaged as a reviewer for the American Journal of Sports Medicine.
Dr. Moulton spent the first three years of his life on the University of Washington campus in Seattle and always thought of Seattle as a second home. He and his wife are excited to now call Seattle home and be closer to the North Cascades, where they enjoy spending much of their free time.

In order to complete any outstanding paperwork and/or process any required changes to your patient information, we ask that you come to our office at least 20 minutes prior to your scheduled visit time.
Please bring the following items to your appointment:
- Photo ID
- A list of your current medications
- Any and all x-rays, MRIs and medical reports available
- Insurance information and card
- Referral or primary care authorization, if your insurance carrier requires it (please check with your carrier)
- Information about what treatments and medications have been tried in the past
- Medical records from prior visits that you think may be helpful
- New patients: please download a patient registration form and bring a completed copy with you for your visit.
