Background
Dr. Chi received his Bachelors degree from Stanford and his medical degree from Harvard Medical School. He did fellowships at the American Sports Medicine Institute in Alabama and at Harborview Medical Center.
Dr. Chi was born in Connecticut and spent the majority of his childhood in California. He’s lived in L.A., the Bay area, Boston, England and Kenya, but says he likes Seattle the best. He and his wife have three kids who keep them busy with their sports and music, and academics. In Dr. Chi’s words, “Sometimes I need to go to work so I can rest!”
In his off time, Dr. Chi is a “former runner, sometime cyclist, hopeful water skier, avid snow skier, and aging third-degree black belt.”

Getting Ready For Your Visit
You can now complete your paperwork ahead of time.
Log in to MyChart now!
In order to complete any outstanding paperwork and/or process any required changes to your patient information, we ask that you come to our office at least 20 minutes prior to your scheduled visit time.
Please bring the following items to your appointment:
- Photo ID
- A list of your current medications
- Any and all x-rays, MRIs and medical reports available
- Insurance information and card
- Referral or primary care authorization, if your insurance carrier requires it (please check with your carrier)
- Information about what treatments and medications have been tried in the past
- Medical records from prior visits that you think may be helpful
- New patients: please download a patient registration form and bring a completed copy with you for your visit.
